Often individuals with ASD demonstrate resistance to change because of impaired flexibility of thought, and the concept of transition is a difficult one. Thus the function of preparing individuals for transition, especially when leaving school is fundamental.
The student’s current and future needs are central to the process of identifying the most appropriate route for the ‘life after Wargrave’.
A number of options will be considered:
Further Education in a mainstream college with an appropriate level of support
Further Education in a specialist college with/without a residential element
Supported living
Employment
Discussion and formulation of the transition plans usually take place in the first year of placement at Lakeside through the annual review procedure. A multi-disciplinary approach is essential and parents, professionals and students are encouraged to consider the options available including undertaking visits to other provisions, often with the support of the school’s Family Liaison Officer.
This transition plan and progress towards the objectives are reviewed and updated formally through the annual review procedure and as necessary throughout the year in conjunction with Connexions, Social Services and others supporting the individual child/young person.
In the final year prior to transition, school staff liaise closely with other provisions to ensure as smooth a transition as possible.

Supporting funding applications
