The Team is committed to working in partnership with parents / carers, colleagues and agencies to secure the best possible outcomes for children, young people and their families.
Natalie Molyneux, Family Liaison Officer
Arrangement of school and college visits for prospective students and advice on admission procedures
Support for students and families with transition and preparation for adulthood, career planning and life beyond Wargrave House
Facilitating opportunities for parents and carers to meet and share experiences and knowledge – often over a cuppa!
Assistance with completing benefit forms, charity applications and support letters
Referral and liaison with external agencies supporting families
Attendance at all annual reviews and social care meetings
Robert Richardson, School Health Nurse
Management of medication within the school, college and LEAP setting.
Health check for all School, Lakeside College and LEAP students
Audiology test on entry to school and Lakeside College
Annual eye tests and a glasses prescription (if required)
Support for parents re. health related appointments for their child e.g. CAMHS
Referrals to relevant health services where a need has been identified, for example, Community Paediatrician.
Health promotion and education within school, college and LEAP including, supporting PSHEE lessons
Andrea Smethurst, SENCO
Liaison with parents/carers, Social Care and Health professionals to prepare and plan for student’s transitions
Ensuring that school, college and LEAP fulfil duties in relation to the SEND Code of Practice: 0-25 years (2015)
Convening and chairing annual and interim review meetings for students in school, college and LEAP
Leading on the review of Education Health and Care (EHC) Plans as required by the SEND Code of Practice: 0-25 years (2015)
Liaison with parents/carers and professionals regarding issues in relation to the provision identified in student’s EHC Plans
Working in partnership with the Family Liaison Officer and School Health Nurse to ensure that the needs of students in school and college are responded to, in accordance with requirements
